Question: Which form do I send for my STATE taxes in WI- 1A or 1040EZ?
I e-filed online, and everything has been accepted by both state and federal gov. I have saved my PDF file for my tax forms. I know I need to sign and send my 8453-OL to the fed. I was going through and found both the 1A and 1040EZ forms in the PDF file (both with signature boxes). The 1A form has addresses of where to mail that form in Madison, WI. On the IRS website there are addresses for the 1040EZ to be mailed all the way to CA? Is that right? Would i send my STATE tax form OUT OF STATE? Do I send out all three (8453,1040EZ, and 1A) to different places? I am single with no dependents (only 19 years old). I cant remember what I did last year. What happens if I send a form that they don’t need? Will they just disregard it? I guess I need to know which forms (8453-OL, 1040EZ, and the 1A) need to be signed and where they need to be sent and to which locations..or if I don’t need to send one of them.
Answer: For WI if you e-filed you should not have to mail anything if expecting a refund unless form W-RA or payment is required. If you owe tax and paying by check, Form EPV should be sent with the check to Milwaukee. http://www.revenue.wi.gov/faqs/pcs/e-faq.html
When federal e-filing do NOT mail your 1040EZ. The 8453-OL should either be sent to the address in the acknowledgement you receive(d), or the address on 2nd page of 8453-OL (Andover, MA).
However, if you owe tax, Form 8453-OL should be included with Form 1040-V and your payment to a different address (Fresno?).
